Portal registration, without the guesswork

Our patient portal registration asks for a fair amount of information — six steps' worth. The good news: with the right things in front of you, it takes about ten minutes, one pass, done. This page shows you exactly what each step asks for, so nothing catches you off guard.

Before you start, gather these

Five minutes of gathering saves twenty minutes of hunting mid-form.

Insurance card

Plan name, member ID, and the policy holder's name — front and back of the card has everything.

Medication list

Names and doses of what you take. A photo of the bottles counts.

Username & password

Decide them ahead of time, plus answers to three security questions — and write them down.

The six steps, decoded

What the portal will ask for, in order.

1 · About you

Your legal name, date of birth, and gender — exactly as they appear on your insurance card.

2 · Contact details

Home address, the phone numbers you use (home / cell / work), and an email address you check regularly — appointment updates go there.

3 · Your account

Pick a username and password, and answer three security questions. Jot your choices down somewhere safe before you start — this is the step where most people get stuck on a later visit.

4 · Insurance

Have your insurance card in hand: the plan name, member ID, and the policy holder’s name (if the plan is in someone else’s name). Self-pay? Just say so — there’s a checkbox for that.

5 · Health basics

Your current medications (names and doses), your preferred pharmacy, and your referring doctor’s name if you have one. A photo of your medication bottles works great as a cheat sheet.

6 · Verify & submit

Type the verification code shown on screen and submit. You’ll use your new username and password to sign in from then on.

Do I have to register on the portal to become a patient?
No. If you'd rather not, send an appointment request on our homepage or call (440) 322-0872 — our team will take your details by phone and help you get set up.
How long does registration take?
About 10 minutes if you have your insurance card and medication list in front of you — that preparation is most of the battle.
What if I get stuck partway through?
Call us at (440) 322-0872 and we'll walk you through it — or just skip the portal and let us handle registration with you by phone.
I already registered but forgot my username or password.
Use the "Forgot password" link on the portal sign-in page, or call the office and we’ll help you reset it.
